Just about everything. Custom motor mounts, the wiring and computer that came with the engine, a transmission to match (again, just try to get the engine, transmission, driveshaft, rear axle and wiring/computers from the exact same car, this will save you TONS of time in the long run). There may be sheet metal mods to be made to shoehorn this engine under the hood. Basically, everything except the body will have to be changed or modified. The cooling system won't cope, the fuel system won't cope, the stock suspension won't cope. But when you are done, it'll be unique, and to me that's worth a ton of extra work.

There are several 5.0 engines that have been installed in b Series pickups. Is your mazda a 2 or 4wd? If you want to run fuel inection the 87-93 mustang with wiring and ecm is only a 5 wire hook up into your mazda. If your truck is a 2wd then there is a fair amount of firewall "adjusting"{ that is necessary. If you have a 4wd a simple 3 inch lift and some work with engine and tranny mounts will do. (also if you use a 5.0, use the accessory brackets and pieces from a 1996-2000 ford explorer 5.0. This will give you about an extra inch in the front.

Have you throught about a rotary 13B or a Mitsubishi 4G63. The 4G63 bolts right up to the B series transmission and can make just as much power as any V8.

I'll say this. If I were trying to decide between a V8 and a built up 4, I'd go with the V8. Not just because it's bigger but because, let's say the small block V8 is putting out 225 hp in stock form. It will do this for a LONG, LONG time because it's very unstressed. That little 4G63 is going to be high-strung because it's soo built. From my experience, built motors are less reliable than stock ones. Another thing, that 4G motor will be putting out, let's say the same 225 hp, but maybe 225 lbs. ft. of torque. The V8 will be belting out more like 300 lbs. of torque.
